Hartford Fellows create materials for National Healthcare Decision Making Day

The Hartford Partnership Program for Aging Education (HPPAE) is a specialized educational program designed to enhance leadership skills in gerontology. Students in the program develop competencies curated to contribute to the social work workforce with the older adult population. Throughout the fellowship, Hartford Fellows create projects to learn more about gerontology and the aging community and present to the School of Social Work and stakeholders.

Hartford Fellows Hannah Somers, Caitlyn Raehse, Camille LaRock, Kamryn Abrial, and Paris Keegan focused on developing a two-pager explaining the importance of Advanced Care Planning conversations. Advanced Care Planning (ACP) is the process of planning for future medical care you may need, in case you become too sick to make your own decisions. Their two-pager shares a roadmap to these difficult conversations and features both resources and prompts for those discussing ACP with their loved ones.

Hartford Fellows Skylar Brown, Cait Sanders, and Laura Zeilo created a toolkit entitled "Advance Care Planning: A Toolkit for Empowerment."
